Take the Mayo Health Assessment

Now is the time to complete the Mayo Health Assessment in order to enhance your wellness awareness, and to qualify for financial and coaching incentives. Take the assessment online at www.elcaforwellness.com. By completing this simple assessment, you will earn $100 for your personal wellness account, and then receive an additional $300 by earning additional reward points.
The ELCA Board of Pensions has continued the incentive that rewards the "employing entities" in each synod with a 2% rebate on the premiums once a synod has achieved a 75% enrollment rate for the assessment. This means, the sooner we have 75% of our eligible plan participants enrolled, the sooner we realize these savings. For the Synod Office, this would mean a monthly savings of over $120! Please remind your spouses and the lay workers in congregational settings who are enrolled in the ELCA Board of Pensions medical plan to take this health assessment as soon as possible.
Editor's note: Bishop Paul took this assessment earlier this week. It took him 15 minutes to complete this process (and he completed it without all the desired info). He now has $100 in his rewards account, and a plan for wellness in the New Year.
The following information will be helpful to have ready when taking the assessment: Weight, blood pressure, low-density lipoprotein (LDL) and high-density lipoprotein (HDL) cholesterol levels, triglyceride level (fasting), and blood sugar level (fasting or non-fasting).
